LIBERAL, KS- Southwest Medical Center (SWMC) is proud to announce a new collaboration with ChartSpan, a leading provider of Chronic Care Management (CCM) solutions. This program went live on Monday, February 3, bringing personalized, preventive care to Medicare patients with two or more chronic conditions, improving health outcomes and reducing hospital readmissions.

Millions of Americans live with chronic conditions such as diabetes, hypertension, and heart disease, making ongoing medical support critical for long-term health. Chronic Care Management (CCM) is a proactive healthcare solution designed to help patients navigate their conditions more effectively, reducing the risk of emergency room visits and hospital readmissions. By partnering with ChartSpan, Southwest Medical Center is expanding access to comprehensive, patient-centered care for Medicare beneficiaries in Seward County and the surrounding areas.

Through this initiative, eligible Medicare patients will benefit from:

24/7 Access to Care: Enrolled patients will have a direct line to support through phone and text, along with monthly check-ins from a dedicated care manager to address health concerns, schedule appointments, and manage medications.
Personalized Care Plans: Patients will receive tailored care plans reflecting their medical history, personal goals, and provider recommendations, empowering them to take control of their health.
Expanded Healthcare Access: This program will serve Medicare patients across Seward County and beyond, connecting them to essential community resources and addressing social determinants of health assessments and directions to local community resources.
